제목 Suicidal Behaviors and Risk Factors for Depression among Adult Stroke Survivors: the KNHANES VI-VII

Introduction
Depression is a common complication after stroke and is associated with increased mortality and greater disability. This study aimed to investigate the suicidal behaviors and to identify the risk factors for a high patient health questionnaire-9 (PHQ-9) score, which is a brief depression severity measure, among adult stroke survivors.

Methods
This study was based on data from the Korea National Health and Nutrition Examination Survey (KNHANES) 2013–2018. A total of 33,226 adults were analyzed. Student’s t- and Chi square tests were used to compare the stroke subjects with controls. Complex samples general linear model was performed to identify the risk factors for a high PHQ-9 score.

Results
Table 1 shows the demographic and clinical characteristics of the study subjects. Mean PHQ-9 score was significantly higher and suicidal behaviors were more common in stroke subjects than in non-stroke subjects (Table 2). Although there was no significant difference in perceived stress level, the distribution of stress causes was different between non-stroke and stroke subjects (Fig 1). Economy, work, and family relationship were common causes of stress in non-stroke subjects. On the other hand, health problems, economy, and marital status commonly caused stress in stroke subjects. Complex samples univariate general linear model showed that male gender, employed status, cohabitating marital status, and high family income was associated with low PHQ-9 scores. The association of gender, marital status, and family income with PHQ-9 score persisted in multivariate general linear model (Table 3).

Conclusion
Stroke was associated with depression severity and suicidal behaviors. Regular psychiatric consultation and appropriate psychological interventions may be needed for stroke survivors.
