Extensive multifocal calcification including cerebral cortex and left coronary artery, aorta, mitral valve, and aortic valve are rarely reported in the absence of common comorbid medical diseases, such as hypoparathyroidism, chronic renal disease or rheumatic diseases.
We serially followed a patient with osteoarthritis and atrial fibrillation who has demonstrated neurologic deteriorations and increased brachial ankle pulse wave velocity in paretic side. A computed tomography (CT) scan revealed multiple calcifications in the right parietotemporal cortex and in the left coronary artery, aorta, mitral valve, and aortic valve. Brachial-ankle pulse wave velocities (baPWVs) of paretic side were aggravated from 26.48 m/s (hospital day: HD 20) to 32.43 m/s (HD 80) and 40.05 m/s (HD 126). Arterial stiffness screened by BaPWV might have contributed to the ischemic cerebral injury and aggravated intracranial calcification which is disturbed by warfarin during the subacute phase of stroke. |
