제목 Effects of self-management at home in patients with recurrent low back pain

Objective: The purpose of this study was to investigate and compare the effects of various self-management programs at home on recurrent low back pain.

Methods: We recruited patients with lower back pain for more than 6 month and an Oswestry disability index(ODI) of 40% or less, and Pain Numeric Rating Scale (PNRS) of between 3 and 6 points. They received pain modality and oral medication(NSAIDs) for two weeks. After that, patients were divided into three groups : 1) Self stretching, 2)Hot pack & TENS, and 3)Thermomassage. Each group was educated with the program that could be performed at home at least 5 days per week for 6 months. We investigate the number of relapses for 6 months and compare with the number of before treatment. The PNRS, ODI, RMDQ (Roland Morris Disability Questionnaire) were evaluated on the baseline, after 2 weeks and after 6 months and compared each groups.

Results: 31 patients (13 male, 18 female) were recruited, mean age was 60.71±8.17 years old.
1) In all three groups, PNRS (from 4.85 to 2.55 at 2week and 1.84 at 6 month), ODI (from 22.73 to 6.06 at 2week, 7.08 at 6 month), and RMDQ (from 12.76 to 3.26 at 2week, 3.38 at 6 month) showed significant decreased.
2) The number of relaps of lower back pain significant decreased in self stretching group (from 1.81 to 0.57) and Thermo-massage group (from 1.86 to 0.64) compared to Hot pack and TENS group (from 1.80 to 1.24).

Conclusions: Self stretching and Thermo-Massage program can be used effective treatment on recurrent lower back pain.
