Background: Stroke patients with hand paralysis may have deterioration of hand function and activity of daily living due to weakeness and spasticity of the hand. Regular grip strengthening exercise can provide patients to improve the grip power and hand function. The game-based rehabilitation system was developed for stroke patients with deteriorated hand function, equipped with a sense to measure grip strength using general hand gripper. The purpose of this study is to increase the participation in exercise by playing a fun PC game using the grip strengthening exercise and to improve the exercise effect through quantitative biofeedback.

Methods: The grip sensing device was established with a combination of force-sensing linear potentiometer sensor (FSLP) placed on the GD gripper which was operated by the microcontroller (Arduino nano). Moreover, the Flying Force game was evolved in the Unity 2019.2.13f1 tool for allowing patients to entertain and improve their grip strength with our grip sensing device that can play in multiplayer mode on one PC. When the patients put a force on the sensing device, the FSLP sensor actives; and a microcontroller reads the force value and transmits to regulate an object in the game via the serial port of PC.

Results: After appling this grip sensing device to stroke patients with hand paralysis, the actual grip strength and their hand function were significantly improved. In addition, it entertained and motivated the patient for exercise.

Conclusions: The grip sensing device is designed to encourage the patients to practice grip strength exercise with fun. In addition, the device can be used at home without complications, and if sufficiently practiced, it can provide an actual functional improvement.
