제목 Correlation between RCSP angle and radiologic parameters in children with flexible flat foot

Purpose: The resting calcaneal stance position angle (RCSPA) is a common biomechanical parameter to diagnose flatfoot. RCSPA reflects subtalar joint position in the frontal plane. However, it is not clear if differences exist between clinical and radiographic assessment of flexible flat foot. The objective of this study was to investigate the correlation between clinical measure of hindfoot alignment and radiologic parameters according to age group in children with flexible flat foot.

Methods: This retrospective study included a total of 453 measurements from 114 children with flexible flat feet (mean age at assessment 7.6 ± 3.0 years old; 66 boys, 48 girls). RCSPA was measured in relaxed standing position. Bilateral weight-bearing foot radiographic assessment was conducted. Radiographic parameters were measured by blinded and independent reader: talometatarsal angle (TMA), talocalcaneal angle (TCA) and calcaneal pitch angle (CPA) on lateral views. Additionally, hallux valgus angle (HVA) was measured on antero-posterior views. Pearson correlation coefficients were calculated to estimate the correlations between clinical and radiographic parameters of flat foot in each age group. P-values of less than 0.05 were considered significant.

Results: The RCSPA showed a weak negative correlation with TMA and TCA (r=0.33, 0.23, respectively; p<0.05), but not with CPA or HVA in pre-school children. As for young teens, RCSPA showed significant correlation with TMA, TCA, CPA, and HVA.

Conclusion: This study investigates how well clinical biomechanical parameters measured by RCPSA relate to the radiologic assessments. TMA and TCA were significantly correlated with RCSPA in all age group. However, RCSPA has a limited role in reflecting sagittal deformities such as calcaneal pitch in preschool aged children.
